Eligibility

Who Can Apply

  • Farmers and landless agricultural labourers of Uttar Pradesh aged 18–70
  • Families with annual income up to ₹75,000 (rural) or ₹1 lakh (urban)
  • Artisans, small traders, and unorganised sector workers in the BPL/EWS category
  • Families listed in the Uttar Pradesh government's beneficiary database
  • Both male and female heads of household are eligible

Who Cannot Apply

  • Persons above 70 years of age
  • Government employees and income tax payers
  • Families with annual income exceeding ₹75,000 (rural) or ₹1 lakh (urban)
  • Non-UP domicile residents

Benefits

  • ₹5 lakh compensation for accidental death or permanent disability
  • Free cashless treatment up to ₹2.5 lakh at empaneled hospitals
  • ₹1 lakh compensation for partial permanent disability
  • Cover for death or injury due to snake bite, animal attack, drowning, electrocution, and road accident
  • No premium to be paid by the beneficiary

  • Aadhaar Card
  • UP domicile certificate or voter ID
  • Income certificate (below ₹75,000 rural / ₹1 lakh urban)
  • Land records or agricultural labourer registration (for farmer category)
  • Bank passbook (Aadhaar-linked)
  • Ration card
  • Passport size photograph

How to Apply

Online

  1. Visit the UP government portal at upagriculture.com or check your district NIC portal
  2. Some districts allow online registration; contact the district agriculture office for current process
  3. For claim filing, visit the nearest empaneled hospital with the beneficiary card and Aadhaar

Offline / CSC

  1. Visit your District Collectorate or Tehsil office
  2. Collect the Mukhyamantri Kisan & Sarvhit Bima Yojana registration form
  3. Fill with personal, income, and land details and attach required documents
  4. Submit to the district lekhpal or revenue officer for verification
  5. On approval, beneficiary details are enrolled for insurance coverage

Frequently Asked Questions

The family or legal heir of the deceased beneficiary files the claim at the district collectorate within 45 days of the accident, along with the FIR, post-mortem report, and death certificate.
